A quick explanation for those wondering about the idea of punching through a skull, the chap question is Judge Fear, one of the Dark Judges from another dimension who prosecute, convict and execute the living for the crime of being alive. The helmet is empty and its contents are the manifestation of the victims fears. So in this case, Dread’s fist only goes through his own fears and the back of the helmet.
